Illinois DSD SB 21: School Bus Driver Physical Exam

Illinois school bus driver permit applicants need this seven-page physical exam form, revised January 2026. The driver completes a health history; the medical examiner records vitals, vision, hearing, urinalysis and a 14-system exam, then certifies the result in two steps: Part A after the exam and Part B once drug and tuberculosis results are in.

Who completes it

The driver completes Section 1 (health history and medications) and signs the Section 4 consent and declaration. The medical examiner completes Section 2 (exam), Section 3 (required testing) and both parts of the Medical Examiner's Certificate with a professional license number.

Submitting and payment

How to submit it
The examiner gives one signed certificate to the applicant, forwards a copy to the employing agency or organization, and keeps one copy with the exam record.

Good to know

  • Annual urine drug testing for marijuana, cocaine, opiates, amphetamines and phencyclidine uses 49 CFR 40.85 cutoffs and must be done no more than 90 days before the permit application.
  • The vision standard is at least 20/40 in each eye and 70 degrees of horizontal field; hearing requires a forced whisper at 5 feet or average loss of 40 dB or less.
  • Under 92 Ill. Adm. Code 1035.20, epilepsy, current alcoholism, and a positive TB test without prophylactic treatment or X-ray-confirmed inactive disease disqualify an applicant.
  • Completing Part A alone does not qualify the driver; the certification date in Part B is the date the examiner received all test results.

Frequently asked questions

Is a TB test required for the Illinois school bus driver physical?

New applicants, and re-applicants whose permits lapsed more than 30 days, need a tuberculosis test. For other re-applicants, the examiner checks for signs of tuberculosis and requires a test if any are present. A positive result is acceptable only with prophylactic treatment or inactive disease shown on X-ray.

How recent must the drug test be for an Illinois school bus permit?

No more than 90 days before the school bus permit application. A copy of the drug testing and control form is kept with the physical exam and certificate, and the examiner must determine whether a legitimate medical explanation exists for any positive result.

Can a driver with insulin-treated diabetes drive an Illinois school bus?

The rules bar insulin-requiring diabetes that is likely to interfere with safe school bus operation. The examiner instructions say that if insulin is needed, the physician must be satisfied the condition is under control and not likely to interfere with the applicant's ability to drive safely.
