Washington DOL Visual Examination Report DR-500-033

Washington's Department of Licensing sends this report when it has reason to believe a driver's vision may affect safe driving. An ophthalmologist or optometrist records acuity with and without correction from an exam within the past year, whether a bioptic was used, horizontal field loss, diplopia, and whether DOL should monitor the driver with periodic reports. DOL alone makes the licensing decision.

Who completes it

The driver, or a parent for a minor, signs the consent to release information. The ophthalmologist or optometrist answers all questions, adds comments on other conditions and signs under penalty of perjury.

Submitting and payment

How to submit it
Return the report to DOL Restricted Licensing by mail, fax or email by the due date DOL writes on the form; failure to return it may result in suspension of the driving privilege.

Good to know

  • Best attainable acuity that is not at least 20/70 with correction is deemed unqualified for night driving.
  • If the total horizontal field is under 110 degrees, record left and right temporal degrees and note any decline in the past 12 months.
  • The doctor can recommend periodic Visual Examination Reports every 6 months, 1 year or 2 years.
  • The report is issued under RCW 46.20.041 and 46.20.305; the related DOL form is the Driver Evaluation Request (DR-500-008).

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Is the Washington Visual Examination Report available in Spanish?

Yes. DOL publishes a Spanish version, Informe del examen de la vista, as well as Russian and Vietnamese versions. The clinician section asks for the same information in each: acuity with and without correction, field of vision, diplopia and monitoring recommendations.

How recent must the eye exam be for DR-500-033?

The driver's consent covers an examination conducted within the past year, and the form asks for a date of examination within the past year. The completed report must reach DOL by the date written on the form, or the driver's privilege may be suspended.
