New Jersey DR-15: School Bus Driver 70+ Medical Exam

New Jersey requires school bus drivers aged 70 and older to have this extra medical examination, yearly from 70 to 74 and every six months from 75. A medical doctor or osteopathic physician examines the driver against the 49 CFR 391.41 standards and signs an evaluation page that goes to the employer, not to the Motor Vehicle Commission.

Who completes it

The driver completes the personal information and health history and signs; a licensed MD or DO completes the examination pages and the Medical Doctor or Osteopathic Physician Evaluation page on page 5.

Submitting and payment

How to submit it
Give only the signed evaluation page to the driver for the employer, who keeps it in the employment records; do not send the form or evaluation page to the MVC, and keep the full DR-15 on file in your office.

Good to know

  • DR-15 does not replace the federal Medical Examiner's Certificate under 49 CFR 391.43; school bus drivers must keep submitting that certificate as well.
  • Only a licensed medical doctor or osteopathic physician can complete it, and the evaluation page asks you to mark MD or DO.
  • Urinalysis is required with numerical readings for specific gravity, protein, blood and sugar; the vision standard is 20/40 in each eye with at least 70 degrees of horizontal field.
  • Drivers who do not comply may have their school bus endorsement suspended, and employers who do not keep the evaluation page face penalties under N.J.A.C. 13:20-30.17.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

How often do New Jersey school bus drivers over 70 need the DR-15 exam?

Drivers aged 70 through 74 need it annually, and drivers 75 and older every six months, under N.J.S.A. 39:3-10.1 and 39:3-10.1a. Each time, the evaluation page goes to the employer to be kept with the driver's employment records.

Can a nurse practitioner complete New Jersey's DR-15?

No. The form must be completed by a licensed medical doctor or osteopathic physician, and the evaluation page asks the signer to mark MD or DO. It is separate from, and in addition to, the federal Medical Examiner's Certificate required by 49 CFR 391.43.

Does the DR-15 get sent to the New Jersey MVC?

No. The form tells drivers not to submit it or the evaluation page to the Motor Vehicle Commission. The evaluation page goes to the employer, and the physician certifies that the complete DR-15 with any attachments is on file in their office.
