Mississippi DR-11: Driver Medical Form

Mississippi's Driver Service Bureau sends this one-page medical form to a driver with a 30-day Notice of Concern after a safety report, and also uses it after an examiner referral. The physician checks the relevant diagnoses, applies the printed seizure waiver criteria, and says whether the driver should lose the privilege immediately or needs further evaluation of driving competency.

Who completes it

The patient signs the authorization in Section A and fills in Section B; the licensed physician completes Section D, signs, and gives the report date and exam date.

Submitting and payment

How to submit it
Only the medical professional's office sends it, by email or mail to the DPS Driver Records Medical Division. The DPS Medical Department page lists a newer mailing address than the one printed on the form.

Good to know

  • No action should be taken on the license if a printed seizure waiver applies: a two-year nocturnal or on-awakening pattern, a two-year prolonged aura with warning, or a provoked seizure after six seizure-free months.
  • If you answer that the driver should lose the privilege immediately, attach a supporting narrative.
  • After review, DPS schedules a driving evaluation, and the examiner relies heavily on the medical professional's opinion. A driver who struggles with the practical test may retake it after six months.
  • The physician's statements are made subject to Mississippi's penalties for unsworn falsification. Type or print in black ink; the current version is revised 06/2025.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How do I submit Mississippi's DR-11 medical form?

The form must come from the medical professional's office, not the driver. The DPS Medical Department page lists email or mail to the Driver Records Medical Division; check that page for the current mailing address, which differs from the one printed on the form. DR-11 arrives with the DE-26 vision form in the driver's Notice of Concern.

What seizure waivers are on the DR-11?

The form lists three: a two-year history of seizures only at night or immediately on awakening; a two-year history of a prolonged aura with sufficient warning; or six seizure-free months where the seizure occurred with a nonrecurring transient illness, toxic ingestion or metabolic imbalance. If one applies, no action should be taken on the license.

How long is the DR-11 valid?

DPS says the DR-11 and DE-26 are valid for one year from the date a licensed medical professional completed them. For a driver who arrives at a license station on crutches, canes or in a wheelchair, or with an amputation, the examiner completes form DR-11A first to start the medical evaluation.
