Utah DLD 49 Certificate of Visual Examination

Utah's Driver License Division relies on this one-page report from a vision health care professional to judge whether a driver's eyesight meets its standards. The examiner records acuity with and without correction, whether each eye meets the 120-degree field standard and, if not, whether fields reach 90 or 60 degrees. The examiner then recommends a review time frame, restrictions and any driver testing.

Who completes it

The driver fills in the identity block and signs; a vision health care professional completes everything below it, including required items such as exam date, printed name, signature and degree.

Submitting and payment

How to submit it
Submit it through the DLD Medical Portal, which Utah prefers, or by email, fax or mail; DLD will not accept it if it is older than 6 months.

Good to know

  • The field standard is 120 degrees, 60 to each side of fixation, and the form notes it is the same for CDL and private drivers.
  • For CDL drivers, the examiner also answers whether the driver is color blind.
  • Recommended restrictions include roads posted 40 mph or less, daylight driving only, and an area restriction, which requires a driving review.
  • Other medical conditions go on DLD 134, the Functional Ability Evaluation, and the Invisible Condition Request uses a vision form for blindness or visual impairment.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.
