North Carolina DL-78: DMV Medical Report Form

North Carolina's DMV Medical Review Unit mails this packet to a driver whose fitness to drive is under review, naming the condition of concern. The physician completes the general history and opinion page plus only the condition sections that apply, such as vision, cardiovascular, diabetes, neurologic or respiratory, and a DMV-approved medical advisor reviews it to make a recommendation.

Who completes it

The driver completes the consent page; a physician licensed in North Carolina or another US state completes the history, opinion and relevant condition sections, signing as MD, NP or PA, and an ophthalmologist or optometrist completes the vision page.

Submitting and payment

How to submit it
Mail or fax the completed form to the Medical Review Unit within 30 days of the driver's letter; if it is not received in time, the license is canceled until the report arrives.

Good to know

  • Both the driver and the medical provider must sign and date the form, or it will not be reviewed.
  • Answer yes or no for each condition on the history page; a yes means completing that entire section, while conditions marked no need no further answers.
  • Attachments may be needed: a room-air arterial blood gas if paO2 is under 60 mmHg, and a CPAP compliance report for the last year if the patient uses CPAP.
  • The opinion asks separately whether the patient is fit to drive a car and a CMV or school bus, and offers restrictions such as daylight only or 45 mph with no interstate.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How long does a North Carolina driver have to return the DL-78?

Thirty days. The Medical Review Unit's cover letter says that if the completed Medical Report Form is not received within that time, the driver license will be canceled until it arrives. Drivers who need more time are told to contact the unit as soon as possible.

Can a nurse practitioner or PA sign the NC DMV Medical Report Form?

The signature line on the general page asks the signer to indicate MD, NP or PA, while the instructions direct the driver to a physician licensed in North Carolina or any other US state. The vision section is completed by a licensed ophthalmologist or optometrist.

Do I have to complete every section of DL-78?

No. Complete the general history and opinion page, then only the sections for conditions marked yes, such as cardiovascular, endocrine, neurologic or substance abuse. The physician also gives an overall assessment of the patient's condition and any medications that may increase driving risk.
