California DMV DL 546A: Physician's Health Report

California requires this physician's health report for two non-commercial roles: driving a house car over 40 feet and working as a driving school instructor. The physician answers 14 yes-or-no health questions on color vision, fields, hearing, limbs, insulin-treated diabetes, heart and lung disease, blood pressure, neurological and psychiatric conditions, lapses of consciousness and drug or alcohol use, then records acuity and blood pressure.

Who completes it

The physician completes the patient information, health questions, visual acuity, blood pressure and certification, signing under penalty of perjury. The driver signs consent to release the medical information.

Submitting and payment

How to submit it
Return the completed report to the applicant, who submits it to DMV. A DMV-use-only block on page 2 is left for the examiner.
Who pays
The form does not list a fee.

Good to know

  • Explain every Yes answer in the space on page 2 or on a separate sheet of paper.
  • Visual acuity must be at least 20/40 in each eye with or without corrective lenses; blood pressure consistently at 140/90 or higher may need further tests.
  • The applicant must submit a new health questionnaire every two years, except driving school instructors, who do so every three years.
  • Don't use it for commercial licensing, which uses the federal MCSA-5875; the applicant's own questionnaire is DL 546.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Is the California Physician's Health Report available in Spanish?

Yes. DMV publishes a Spanish edition, DL 546A SP, titled Reporte Médico Sobre La Salud, alongside the English form. Both cover the same health questions, vision and blood pressure readings, and certification for house car drivers over 40 feet and driving school instructors.

Can DL 546A be used for a commercial driver's license?

No. The form states at the top that it should not be used for commercial licensing requirements. Commercial drivers use the federal Medical Examination Report, MCSA-5875. DL 546A covers only house cars over 40 feet and driving school instructors.
