California DMV DL 4063: IID Medical Exemption

Drivers ordered to install an ignition interlock device in California can ask for a medical exemption if a condition keeps them from giving the required breath sample. The provider most familiar with their health certifies that they can't meet the minimum of 1.5 liters of breath for the initial test and random retests, and DMV uses the form to decide eligibility.

Who completes it

The driver completes and signs Sections 1 and 2, the driver information and acknowledgement and consent. The medical provider completes Section 3 with name, specialty, license number and contact details, and Section 4, initialing the opinion and signing under penalty of perjury.

Submitting and payment

How to submit it
Email the original to the DMV exemption address printed on the form, or return it to the driver to submit. An email address is required on the form for electronic submission, and you may keep a copy.
Who pays
The form lists no fee.

Good to know

  • If DMV approves the exemption, the driver gets no restricted driving privilege and has only a suspension or revocation option, which the driver acknowledges in Section 2.
  • All information on the form is mandatory, and DMV may deny the request if required information is missing.
  • Exemptions for non-medical reasons use a different form, DL 4062; the current medical edition is DL 4063 (REV. 3/2024).
  • Questions about exemption eligibility go to DMV's Mandatory Actions Unit, listed at the bottom of the form.

Frequently asked questions

What does the medical provider certify on DL 4063?

That, based on the driver's health history and current condition, they are unable to meet the minimum breath sample needed to operate an ignition interlock device: 1.5 liters for the initial test and for random retests while driving. You initial that statement and sign under penalty of perjury.

What happens if a California IID medical exemption is approved?

The driver is exempt from installing the device but will not be granted a restricted driving privilege. Instead, only the suspension or revocation option remains. The driver acknowledges this in Section 2 before the provider completes the medical sections.
