PennDOT DL-129 Loss of Consciousness Reporting Form

Pennsylvania clinicians report a driver's loss of consciousness or awareness on this PennDOT form. PennDOT lists periodic episodes of loss of consciousness and/or awareness among its physical criteria, and the Bureau of Driver Licensing reviews the report against 67 Pa. Code Chapter 83 to decide whether the driver may keep driving, needs a restriction or exam, or faces recall.

Who completes it

The treating physician or other health care provider who evaluated the episodes completes the whole form.

Good to know

  • Seizure disorder and epilepsy have their own PennDOT form, DL-121, which also carries the single-seizure reporting rule in 67 Pa. Code 83.4(c).
  • For school bus drivers, PennDOT uses DL-129SB, Loss of Attention, Awareness, or Consciousness of Unknown Etiology.
  • PennDOT reviews reports within 15 days; a driver judged medically incompetent receives notice that the privilege is recalled seven days from the mail date.
  • The PDF opens only with a PennDOT medical reporting login, requested through the Medical Reporting Forms page.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

When does PennDOT want loss of consciousness reported?

PennDOT's criteria list periodic episodes of loss of consciousness and/or awareness. Providers must report, within 10 days, a patient diagnosed with a condition that could impair safe driving, unless the condition is expected to last less than 90 days. Seizures are reported on DL-121 instead.

What is the school bus version of DL-129?

DL-129SB, titled Loss of Attention, Awareness, or Consciousness of Unknown Etiology, is the Chapter 71 form for school bus drivers. DL-129 is used for drivers reviewed under Chapter 83, which covers drivers other than school bus drivers.

What outcomes can follow a DL-129 report?

PennDOT may take no action, add a license restriction, request more medical information from the driver, who then has 45 days to comply, require a driver's exam, or recall the license until medical competency is demonstrated.
