PennDOT DL-120 Cardiovascular Medical Report Form

This PennDOT condition-specific report covers cardiovascular and cerebrovascular disease, such as conditions with syncope, loss of consciousness, vertigo or dyspnea, in a driver who may not meet Pennsylvania's medical standards. The Bureau of Driver Licensing checks it against 67 Pa. Code 83.5 and Chapter 83, then decides on a restriction, a driver's exam, recall of the license or no action.

Who completes it

The treating physician or other health care provider managing the driver's cardiovascular condition completes the whole form; there is no patient section.

Submitting and payment

How to submit it
Send it to PennDOT's Bureau of Driver Licensing by mail, fax or email, or complete and sign it through the online version linked from PennDOT's Medical Reporting Forms page.

Good to know

  • PennDOT lists cardiovascular conditions and, separately, cerebral vascular insufficiency among the physical criteria it reviews.
  • When PennDOT needs more information, it sends the driver the relevant forms and allows 45 days; a driver who does not comply has the license suspended within 30 days.
  • School bus drivers with a cardiovascular condition are handled on DL-705, the Chapter 71 Cardiovascular Waiver form, rather than DL-120.
  • Opening the PDF requires PennDOT medical reporting credentials, though an online version can be completed and signed instead.

Medical Cardiovascular Form

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Which cardiovascular conditions does PennDOT want reported?

PennDOT's criteria list cardiovascular conditions and cerebral vascular insufficiency, and DL-120 is built around 67 Pa. Code 83.5, which addresses disease with syncope, loss of consciousness, vertigo or dyspnea. Reporting is required for a condition that could impair driving, but not one expected to last less than 90 days.

How fast must a Pennsylvania provider report a heart condition?

Within 10 days, in writing, giving the driver's full name, address and date of birth. PennDOT says including the condition and specific details helps its licensing decision, and it prefers the condition-specific form, here DL-120, over a general report.

Is there a school bus version of DL-120?

School bus drivers fall under 67 Pa. Code Chapter 71, and PennDOT's forms page lists DL-705, the Cardiovascular Waiver, for them. DL-120 applies to drivers reviewed under Chapter 83, which covers drivers other than school bus drivers.
