PennDOT DL-123 General Medical Reporting Form

When a condition affecting safe driving does not fit PennDOT's other condition-specific forms, Pennsylvania clinicians report it on this general medical form, listed by PennDOT as Other Conditions That Affect Safe Driving. The Bureau of Driver Licensing reviews it against 67 Pa. Code Chapter 83 to decide whether the driver meets the state's medical standards.

Who completes it

The treating physician or other health care provider completes the whole form, describing the condition and its effect on driving.

Submitting and payment

How to submit it
Mail, fax or email it to PennDOT's Bureau of Driver Licensing, or complete and sign the online version; reporting providers have 10 days to report in writing.

Good to know

  • PennDOT has separate forms for cardiovascular (DL-120), seizure (DL-121), diabetes (DL-122), neurological (DL-124), orthopedic (DL-126), substance use (DL-128), loss of consciousness (DL-129), cognitive (DL-131) and psychiatric (DL-104) conditions.
  • For school bus drivers, the Chapter 71 equivalent is DL-123A, which PennDOT also lists for respiratory dysfunction and tuberculosis.
  • DL-13, the Initial Reporting Form, is the option for emergency rooms or clinicians without enough knowledge of the condition to complete a specific form.
  • PennDOT may respond by requiring a driver's exam, which includes a vision screening, a knowledge test and/or a skills test.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

When should I use DL-123 instead of DL-13?

PennDOT prefers a condition-specific form. DL-13, the generic Initial Reporting Form, is meant for emergency rooms or providers who do not know enough about the condition to complete a specific form. DL-123 is the specific form for conditions that affect safe driving but are not covered by another PennDOT medical form.

Who must report a medical condition to PennDOT?

Physicians, chiropractors, physician assistants and certified registered nurse practitioners must report, in writing within 10 days, any patient over 15 diagnosed with a condition that could impair the ability to drive. Conditions expected to last less than 90 days are excluded.

Is a PennDOT medical report covered by HIPAA?

PennDOT states that HIPAA regulations do not apply to medical reporting to PennDOT and that no individual consent is needed. Reports are confidential, used only to determine whether the driver is medically fit to drive, and reporting providers are immune from civil or criminal liability.
