Michigan DA-88: Request for Driver Evaluation

Under MCL 257.320, Michigan's Department of State may reexamine a person it has reason to believe is unsafe to drive because of incompetence or a mental or physical infirmity. This two-page request, revised October 2026, is how a clinician, police officer, family member or other concerned person asks for that reexamination by describing a medical episode, incident or pattern of behavior.

Who completes it

The requestor completes every section: the driver's name, date of birth and license number if known, the reason for the evaluation, and their own name, relationship to the driver, contact details and signature.

Submitting and payment

How to submit it
Send the signed form to the Department of State's Driver Assessment Section by mail, fax or email. Email attachments must be PDF, JPEG or PNG files without password protection.

Good to know

  • The Department cannot act unless the request gives specific information showing an unsafe driving condition may exist; extra pages and documents may be attached.
  • Anonymous requests are not accepted, but a private citizen's identity is kept confidential to the extent Michigan and federal law allow.
  • Submitting false information about a person may bring civil or criminal penalties, and the requestor certifies the information is true to the best of their knowledge.
  • If a reexamination follows, the Department mails the driver medical forms such as DA-4P and DA-4V to complete with their physician or vision specialist.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Can a doctor request a driver evaluation in Michigan?

Yes. The Department of State accepts DA-88 requests from the medical community, law enforcement, Secretary of State staff, the legal community, adult protective services, concerned citizens, friends and family. The request must describe the episode, incident or behavior that suggests the person may not drive safely.

Will the driver know who filed the DA-88?

The Department does not accept anonymous requests, but it keeps a submitter's information confidential to the extent permitted by Michigan and federal law, to protect privacy and prevent retaliation. The requestor still signs and gives contact details so the Department can follow up.

What happens after a DA-88 is filed in Michigan?

If the Department schedules a reexamination, it mails a Notice to Appear and any required medical forms to the driver's address on file. Reviews for medical concerns are held in person with a Driver Assessment analyst and may include vision screening, a knowledge test and an on-road test.
