MCSA-5872: Non-Insulin Diabetes Assessment (Optional)

FMCSA has no standard specific to non-insulin-treated diabetes, but certified medical examiners can use this optional four-page form, revised 1/26/24, to ask the driver's treating provider for details. The answers on medications, monitoring, glucose control, hypoglycemic and hyperglycemic episodes, HbA1c, complications and retinopathy help the examiner judge whether the driver can operate a commercial vehicle safely.

Who completes it

The certified medical examiner fills in and signs the request on page 1. The driver's treating health care provider answers questions 1 to 11 and signs with license number and state.

Submitting and payment

How to submit it
The provider returns the form to the requesting medical examiner, either through the driver or by the mail, email or fax details the examiner lists on page 1.

Good to know

  • Question 2 asks for every diabetes medication with dosage and start date; questions 3 and 4 cover glucose testing frequency and compliance with the individualized treatment plan.
  • Questions 6 and 7 ask about recent severe hypoglycemia and significant hyperglycemic episodes such as diabetic ketoacidosis or hyperosmolar syndrome, with dates and details.
  • Attach the most recent HbA1c result if it has been measured over the last 12 months, and give the date of the last eye examination.
  • Using the form is optional for the examiner. Drivers who take insulin need the separate MCSA-5870 completed by the clinician who prescribes their insulin.
