Federal Drug Testing CCF: MRO Steps 6 and 7

Every federally regulated urine or oral-fluid drug test, including DOT tests for FMCSA, FAA, FRA, FTA, PHMSA and USCG employers, travels with this five-part chain-of-custody form. Collectors and the laboratory complete the custody steps, and the Medical Review Officer uses Copy 2 to record the verified result for the primary specimen and, when tested, the split specimen.

Who completes it

The collector or employer representative completes Step 1, the collector Steps 2 to 4, and the donor signs the Step 5 certification on Copy 2. The Medical Review Officer, a licensed physician, completes Step 6 (primary specimen) and Step 7 (split specimen) on Copy 2.

Submitting and payment

How to submit it
The MRO reports the verified result to the employer's designated employer representative. The five copies go to the test facility (Copy 1), MRO (Copy 2), collector (Copy 3), employer (Copy 4) and donor (Copy 5).

Good to know

  • Step 6 verification choices are negative, positive (naming the drugs), dilute, refusal to test (adulterated, substituted or other reason) and test cancelled, with a remarks line.
  • Step 7 records whether the split specimen reconfirmed or failed to reconfirm the result for specific drugs, or whether the test was cancelled.
  • Real forms are pre-printed with specimen ID numbers and supplied by HHS-certified laboratories; HHS-approved electronic CCFs (ECCF) are allowed for DOT tests.
  • The current edition is OMB No. 0930-0158, Version C dated 6 May 2020, and covers both urine and oral-fluid collections.

If your copy of the form is a fillable PDF, upload it once and Vero Documents fills it from the visit note and the chart, ready for you to review and sign. Download, print, or email the finished form, with the patient’s consent.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Can the donor list prescription medications on the CCF?

Step 5 tells donors the MRO may contact them about prescription and over-the-counter medications, and that a list is not necessary. If donors choose to make one, they write it on a separate sheet or on the back of their own Copy 5, never on any other copy of the form.

Who can sign Step 6 of the Federal CCF?

Only the Medical Review Officer, a licensed physician who reviews and verifies the laboratory result. Step 6 asks for the MRO's signature, printed name and date beneath the verification, stated as made in accordance with applicable federal requirements. Step 7 has the same signature block for the split specimen result.

Where can I get blank Federal CCFs?

Federal CCFs used for real tests are pre-printed with a unique specimen ID number that matches the bottle labels and seals, and HHS-certified laboratories supply them. The SAMHSA posting is a proof showing the layout of all five copies. An HHS-approved electronic CCF can be used instead for DOT tests.
