USCG CG-719K/E: Mariner Medical Short Form

Mariners holding only entry-level or staff officer endorsements can use this five-page short form when they want to serve as food handlers or need an STCW or MLC medical certificate without standing navigational or engineering watches. The practitioner records identity, food handler status, physical information and a demonstration of physical ability, then recommends certification to the National Maritime Center.

Who completes it

The mariner completes Sections I and VI and any optional consents in Section VII. A physician, PA or NP licensed by a U.S. state, possession or territory completes Sections II to V and initials and dates each page.

Submitting and payment

How to submit it
The practitioner returns the signed form to the mariner, who submits it to the National Maritime Center; food handlers include the instruction page with Sections I, II, V and VI.

Good to know

  • No lookout duties are authorized on a short-form certificate. Anyone standing navigational or engineering watches, and all other applicants, must use the full CG-719K.
  • For the food handler attestation, consider recent Salmonella Typhi, Shigella, Shiga-toxin E. coli or hepatitis A, acute GI symptoms, and draining lesions on hands or arms; test only if clinically necessary.
  • The physical ability table includes climbing vertical ladders, stepping over a 24-inch doorsill, lifting 40 pounds, standing up to four hours, handling fire hoses and donning an exposure suit.
  • Applicants under 18 need a notarized parent or guardian statement. The 12/2025 edition carries OMB 1625-0040, expiring 09/30/2028.

What recommendation does the CG-719K/E summary ask for?
