Nebraska Form 458B: Homestead Disability Certification

Nebraska homestead exemption applicants who qualify through disability rather than age attach this certification to Form 458 and Schedule I. A qualified licensed medical practitioner checks whether the applicant is a wartime veteran totally disabled by a non-service-connected illness or accident (category 2) or has a qualifying permanent mobility or arm disability (category 3). The county assessor uses it to decide eligibility.

Who completes it

The applicant fills in name, county, address and Social Security number and signs a medical-information release. A physician, PA or APRN completes lines a through f, signs, and adds an NPI; category 6 (developmental disability) is certified only by the Deputy Director of the DHHS Division of Disability and Aging.

Submitting and payment

How to submit it
Return the signed form to the applicant, who attaches it to Form 458 and Schedule I and files them with the county assessor after February 1 and on or before June 30.
Who pays
Form 458B lists no fee, billing code or state payment for the practitioner's certification.

Good to know

  • The disability must exist on January 1 of the application year, and the form certifies only the applicant, not a spouse or another owner-occupant of the home.
  • Category 3 lines cover lost mobility so that locomotion requires a mechanical aid or prosthesis, permanent partial disability of both arms over 75%, or amputation of both arms above the elbow.
  • Receiving SSDI does not qualify an applicant by itself; Nebraska decides categories 2, 3 and 6 without regard to SSDI status.
  • If a medical practitioner signs the category 6 section, the application is denied. The county name must be filled in to authorize the health-information release.
