Utah TC-842: Disabled Person and Physician Certificate

Utah's TC-842 is the certification behind disability parking placards and disabled plates from the Utah DMV. A licensed physician, physician assistant, physical therapist or nurse practitioner certifies that the applicant meets one of six criteria that limit walking. Plates require a permanent disability, while placards can be permanent or temporary.

Who completes it

The applicant supplies their information, and a licensed physician, physician assistant, physical therapist or nurse practitioner completes the disability certification. Care facilities that transport disabled people complete Section 2, the Disabled Person Care Facility Certification.

Submitting and payment

How to submit it
The applicant presents TC-842 at a Utah DMV office or mails it to the Motor Vehicle Division's mail and correspondence unit.
Who pays
Paper placards are free to issue and replace; durable placards carry a per-placard fee, with a limit of two.

Good to know

  • Each case must meet one of six criteria, among them inability to walk 200 feet without resting, needing a brace, cane or wheelchair, portable oxygen, and lung, cardiac or orthopedic limits.
  • Temporary placards last up to six months and cannot be renewed; another temporary placard needs a new certification from the clinician.
  • Permanent placards issued after July 1, 2024 do not expire, and permanent placards and plates need no recertification after the initial one.
  • A person may have one disabled plate per registered vehicle; without disabled plates, they may hold up to two placards.

Frequently asked questions

Who can sign Utah form TC-842?

The Utah DMV says TC-842 must be completed by a licensed physician, physician assistant, physical therapist or nurse practitioner. The clinician certifies that the person meets at least one of the state's six criteria for a disability that limits walking, which is required for any placard or plate.

How long does a Utah temporary disability placard last?

Up to six months, and it cannot be renewed. If the patient still needs a placard after it expires, a new TC-842 certification from the clinician is required for another temporary placard. A condition that turns out to be permanent can be certified as permanent instead.

Do Utah permanent placards need a new doctor's certification?

No. Only the initial certification is needed for permanent placards and plates. Permanent placards issued after July 1, 2024 do not expire; older ones generate DMV renewal notices that the holder signs and returns without a new medical certification.
