NJ SP-41: Disability Plates and Placard Application

New Jersey residents use SP-41 for initial, recertification or replacement wheelchair-symbol license plates or a placard for persons with a disability. In Section E a medical practitioner certifies, after personally examining the patient, which of seven permanent qualifying conditions applies. The Motor Vehicle Commission's Special Plate Unit reviews the mailed application.

Who completes it

The person with a disability and the registered vehicle owner complete Sections A through D and sign Section F. A medical practitioner completes Section E, or a U.S. Department of Veterans Affairs representative signs for 100% permanently disabled veterans.

Submitting and payment

How to submit it
The applicant mails the application, with the practitioner's prescription or letterhead certification and a registration copy for plates, to the MVC Special Plate Unit in Trenton within 60 days of the medical certification.
Who pays
The form does not address payment for the practitioner's certification.

Good to know

  • Besides Section E, the applicant must attach a certification on the practitioner's prescription or official letterhead, with professional identification, under N.J.A.C. 13:20-9.1(a)(4).
  • Conditions include limb loss, permanent need for an assistive device, lung disease thresholds or portable oxygen, Class III/IV cardiac disease, and severe permanent walking limits or inability to walk 200 feet.
  • Plates are renewed yearly with disability recertification every three years; the ID card and placard are recertified and renewed every three years.
  • Permanent sight impairment of both eyes qualifies for a placard only and must be certified by the NJ Commission for the Blind. Temporary needs use SP-68 instead.

Frequently asked questions

What extra document does New Jersey SP-41 require from the practitioner?

Besides signing Section E, the practitioner must give the applicant a certification statement on a prescription or official letterhead that certifies the qualifying disability and includes the practitioner's professional identification, as required by N.J.A.C. 13:20-9.1(a)(4). The applicant attaches it to SP-41.

How long does a New Jersey SP-41 certification last?

The application must reach the MVC within 60 days of the date the practitioner or VA representative certifies the disability. Once issued, the ID card and placard are recertified every three years, and plates renewed yearly need disability recertification every three years.

Can SP-41 affect a New Jersey commercial driver license?

It can. Section A asks CDL holders to acknowledge that the application may result in a medical review, which could lead to a decision affecting their New Jersey CDL privilege. The practitioner's certification is part of what the MVC reviews.
