NY MV-664.1MP: Metered Parking Waiver Application

New York drivers with severe disabilities use MV-664.1MP to apply for a metered parking waiver, which lets them park at any meter in New York State without paying. A medical professional certifies two things: a permanent severe disability under Vehicle and Traffic Law Section 404-a (Part A), and a limitation that makes paying a meter very difficult (Part B). The applicant files it locally.

Who completes it

The person with the disability, or a parent or guardian, completes and signs the top section. Only a physician, physician assistant, nurse practitioner, podiatrist or optometrist completes the Medical Certification, Parts A and B.

Submitting and payment

How to submit it
The applicant brings the completed application and their New York State driver license to the city, town or village office where they live; the DMV itself does not issue the waiver.
Who pays
The form does not address payment for the medical certification.

Good to know

  • Part B requires at least one: limited fine motor control of both hands, inability to reach or access a meter due to a wheelchair or other device, or inability to reach 42 inches high.
  • Applicants need a valid New York State driver license and must live in the issuing municipality. New York City residents are not eligible, because the city's own parking permit covers meters.
  • The medical professional must sign and date the form with an original signature, and certifies under the threat of civil penalty and criminal prosecution for false statements.
  • The waiver works only when the holder is driving without a passenger able to pay. In meter spaces reserved for people with disabilities, the regular permit or plates must also be displayed.

Frequently asked questions

Who can sign New York form MV-664.1MP?

The medical certification may be completed only by a physician, physician assistant, nurse practitioner, podiatrist or optometrist. They check the qualifying condition in Part A and the meter-related limitation in Part B, give a license number, and sign with an original signature.

Can New York City residents get a metered parking waiver?

No. New York City does not issue the Metered Parking Waiver, because its own city parking permit already lets permit holders park at city meters without paying. NYC residents apply for the city permit through the NYC Department of Transportation instead.

Where is the New York metered parking waiver issued?

By the city, town or village office where the applicant lives, not by the DMV. Applicants contact their local office to find out where to apply, and Nassau County residents are directed to a county contact to learn where to apply.
