DC DMV Disability Parking Tags and Placard Application

The District of Columbia DMV uses one form to issue or renew disability parking tags (license plates), a temporary or long-term placard, or both. Most applicants need a medical practitioner to answer four questions about mobility and describe the impairment in detail. People missing a lower extremity or using a motorized wheelchair can self-certify in person instead.

Who completes it

The applicant completes the personal section and signs. Unless the applicant self-certifies at a DMV Service Center, a medical practitioner completes the lower section: the mobility questions, temporary end date or long-term status, a detailed description, and signature with name, phone and practitioner ID number.

Submitting and payment

How to submit it
With a practitioner's certification, placard requests go through DMV's online form or by fax, and tag requests are mailed with payment. The application must be postmarked, hand-delivered or emailed within 60 calendar days of the practitioner's section.
Who pays
Placards are free; tags carry a DMV fee paid by check or money order, and no fee is set for the practitioner's certification.

Good to know

  • The practitioner answers whether the person has a mobility impairment, needs mobility aids such as a wheelchair, walker, crutches, cane or long leg braces, or has a respiratory or other disease that limits mobility.
  • A long-term placard can last up to 8 years, and renewal needs a new certification. For a temporary impairment the practitioner gives an end date.
  • A designated driver may display the placard only when the applicant is a passenger; tags and placards are non-transferable.
  • The form is available in English, Spanish, Chinese, French, Vietnamese and Korean. DC's one-week placard (DMV-ODPPA-01) needs no medical certification.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
