Metro St. Louis ADA Professional Verification Form

Metro Transit and St. Clair Transit District provide ADA paratransit, Metro Call-A-Ride in Missouri and Alternative Transportation Service (ATS) in Illinois, for people who cannot use MetroBus or MetroLink. Every applicant, new or recertifying, must include written professional verification of disability. The blue Professional Verification Form is that document, and an in-person functional assessment follows.

Who completes it

The applicant completes the yellow application forms. The healthcare provider completes the blue Professional Verification Form (AD250323-C), giving name, title, specialization and a professional license number if applicable, and signs.

Submitting and payment

How to submit it
The verification goes with the application by U.S. mail, fax or upload at MetroStLouis.org/ADAUpload. The applicant then calls Metro to schedule the functional assessment.
Who pays
The packet does not address payment for completing the professional verification.

Good to know

  • For primary and secondary conditions, the provider gives DSM-5 or ICD-10 codes if available, onset date, prognosis, expected duration, variability, medication control and how the condition blocks bus or rail use.
  • Condition pages cover epilepsy (seizure frequency, type, aura), cognitive or psychiatric limits, cardiac disease (AHA class and heat or cold precautions) and low vision (corrected acuity, fields, lighting).
  • Metro notes that eligibility is a transportation decision, not a medical one, and that Metro may verify the validity of the professional providing the certification.
  • Decisions come no later than 21 days after the assessment. Presumptive eligibility applies if Metro causes a delay in scheduling or deciding.

Frequently asked questions

Who can complete the Metro St. Louis Professional Verification Form?

The form lists audiologists, rehabilitation specialists, physicians, optometrists, physician assistants, social workers, RNs and LPNs, physical, occupational and speech therapists, orientation and mobility specialists, psychologists and case managers, with an 'Other' option. The professional states how they know the applicant and for how long.

Do recertifying Call-A-Ride riders need new professional verification?

Yes. Metro requires all applicants, whether new or applying for recertification, to complete a new application and provide written professional verification of disability. Recertifying riders go through the same in-person functional assessment as new applicants.

What happens after the professional verification is submitted?

The applicant calls Metro to schedule an in-person functional assessment, about seven days after mailing or two days after faxing or uploading. Metro can arrange the ride to the assessment and gives its decision no later than 21 days after the appointment.
