Sun Life Prior Authorization Drug List and Forms

Sun Life group plans need approval before covering drugs on their prior authorization list, with drug-specific request forms hosted by Express Scripts Canada. The plan member enters a group contract number on Sun Life's page to confirm the plan includes prior authorization. Decisions rest on pre-defined clinical criteria, mainly Health Canada approved indications and evidence-based protocols.

Who completes it

The plan member completes Part A (patient information, coordination of benefits and signed authorization) and the prescribing physician completes Part B, covering the drug requested, eligibility criteria and prescriber information.

Submitting and payment

How to submit it
Upload all pages through the my Sun Life mobile app or website using 'prior auth' as the reference number, or fax or mail them to Sun Life's claims office in Montreal or Waterloo.
Who pays
The instructions state that any fees related to completing the form are the plan member's responsibility.

Good to know

  • Part B warns that incomplete forms may result in automatic denial and asks prescribers not to provide genetic test information or results.
  • Drugs used for indications not approved by Health Canada may be denied, and for Quebec plan members RAMQ exception drug criteria may apply.
  • Renewal requests should include proof of prior coverage if available; some forms set renewal criteria, such as at least a 50% reduction in monthly migraine days for Aimovig.
  • If approved, Sun Life mails an approval letter; members in its Preferred Pharmacy Network also get a call from Express Scripts Canada about their coverage.

Fill it with Vero

If your copy of the form is a fillable PDF, upload it once and Vero Documents fills it from the visit note and the chart, ready for you to review and sign. Download, print, or email the finished form, with the patient’s consent.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

How do I get a Sun Life prior authorization form?

On Sun Life's Prior Authorization Drug List and Forms page, enter the group contract number to confirm the plan includes prior authorization, then choose the drug's form. If a form does not load, Sun Life suggests trying another browser or saving the file and opening it from the computer.

What does Sun Life need from the prescriber?

Part B asks whether the request is new or a renewal, then the DINs, dose, route, frequency, duration and site of administration. Next come the drug-specific eligibility criteria, or a note that none apply with additional information. The prescriber adds licence number, specialty and signature.

Does Sun Life cover the drug if a provincial plan could pay?

The form says drugs in the program may be eligible for reimbursement if the patient does not qualify under a primary plan or government program. Part A asks whether the patient applied to a primary plan and for its decision letter, and tells patients to apply to the province where provincial coverage may exist.
