NB Smoking Cessation Special Authorization (917E)

New Brunswick Drug Plans uses this one-page form to request additional smoking cessation therapy for a patient: either more bupropion tablets or nicotine patches, but not both. Additional bupropion is capped at 168 tablets a year, and additional patches are restricted to people taking part in the Ottawa Model. The requestor records how many cigarettes the patient smoked per day before therapy and smokes now.

Who completes it

The prescriber (requestor) completes all five sections, from requestor and patient details to the therapeutic information and the signature with licence or registration number.

Submitting and payment

How to submit it
Fax the form to the Special Authorization Unit of New Brunswick Drug Plans, or mail it to the unit in Moncton.

Good to know

  • Additional bupropion is 150 mg twice daily, with a maximum of 168 additional tablets permitted annually, and requires a statement that the person has a high probability of quitting.
  • Additional nicotine patches are restricted to people in the Ottawa Model; the form asks for the participating hospital, clinic or health centre.
  • For patches, give the number of patches per day and anticipated duration, plus cigarettes per day before starting nicotine replacement and currently.
  • Forms missing information are returned. The French version is FORM-917F, and correctional facilities use FORM-1140E, which offers bupropion only.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Can a patient get both bupropion and nicotine patches on the NB form?

No. FORM-917E states that requests are considered for bupropion tablets or nicotine patches, not both, so the requestor selects one. Bupropion is limited to 168 additional tablets a year, and additional patches are restricted to people participating in the Ottawa Model.

What is the Ottawa Model requirement on New Brunswick's smoking cessation form?

Additional nicotine patches are restricted to people participating in the Ottawa Model, and the form asks for the name of the participating hospital, clinic or health centre. The requestor also gives the number of patches per day, expected duration and cigarettes smoked daily before starting nicotine replacement and currently.

Is New Brunswick's smoking cessation special authorization form available in French?

Yes. 'Thérapie d'abandon du tabac - Demande d'autorisation spéciale' is FORM-917F, the French counterpart of FORM-917E. The English form was revised in April 2024 and is faxed or mailed to the Special Authorization Unit of New Brunswick Drug Plans.
