NB Paxlovid Special Authorization Form (FORM-1238E)

New Brunswick Drug Plans covers nirmatrelvir/ritonavir (Paxlovid) for adults who meet set clinical criteria, and this form documents them. Unlike other New Brunswick special authorization forms, it is not faxed to the plan: it stays with the prescription at the pharmacy, which bills electronically using the intervention code that matches the criterion selected.

Who completes it

The prescriber (requestor) completes all five sections, including the therapeutic criteria and the signature with licence or registration number; the pharmacy keeps the completed form with the prescription.

Submitting and payment

How to submit it
Give the completed form to the pharmacy with the prescription; it must not be faxed to New Brunswick Drug Plans, and the pharmacy submits the claim electronically with the matching CPhA intervention code.

Good to know

  • The patient must be 18 or older and either have tested positive with symptoms in the last 5 days, or hold a future-use prescription to fill only under those conditions.
  • Severe immunosuppression (solid organ or stem cell transplant, hematologic cancer treatment, anti-CD20 therapy in two years) maps to intervention code UT; moderate immunosuppression maps to VE.
  • Moderate immunosuppression includes cancer treatment, recent biologics or immunosuppressing drugs, advanced HIV, and renal conditions such as dialysis or eGFR below 15 mL/min/1.73 m².
  • Dose and regimen are limited to a maximum of 5 days. A French version (FORM-1238F) exists, and correctional facilities use FORM-1239E.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Do I fax the NB Paxlovid special authorization form?

No. The form states it must be completed and retained with the prescription at the pharmacy and asks prescribers not to fax it to New Brunswick Drug Plans. The pharmacy submits the claim electronically, including the CPhA intervention code that matches the clinical criterion checked.

Can I write a Paxlovid prescription for future use in New Brunswick?

The form allows it: the requestor can indicate the prescription was written for future use, and the patient fills it only after testing positive for COVID-19 and within 5 days of symptom onset. The patient must still be 18 or older and meet one of the immunosuppression criteria.

Who qualifies on the NB Paxlovid form?

Adults 18 or older who are severely immunosuppressed, for example after a solid organ, bone marrow or stem cell transplant, or moderately immunosuppressed, for example during cancer treatment, on significantly immunosuppressing drugs, with advanced HIV, or with renal conditions such as dialysis. Each group has an 'Other' option to specify.
