NL MRD Neurology Report Form: Seizures and Driving

Newfoundland and Labrador's Motor Registration Division requests this one-page report to decide fitness to drive when a driver has epilepsy, a seizure or another neurological condition. The physician or nurse practitioner records seizure dates, alcohol or drug involvement, medication, adherence and EEG results, then states whether the condition or its treatment affects the patient's ability to operate a motor vehicle.

Who completes it

The physician or nurse practitioner answers questions 1 to 8 and adds name, address, signature and examination date; the driver enters name and licence number and signs to authorize release of the information.

Submitting and payment

How to submit it
The form prints no return instructions, so send it to MRD's Medicals and Driver Records section, which lists fax, email and mailing contacts on MRD's contact page.
Who pays
No fee is printed on the form, and MRD's related medical letters say the driver may be responsible for any costs.

Good to know

  • Question 4 asks for an EEG report and results, so have the most recent EEG on hand before completing the form.
  • For provoked seizures, the form asks whether alcohol or drugs were involved and since when the patient has abstained from the seizure-causing substance.
  • Question 2 asks whether the last seizure followed stopping or changing prescribed medication, and question 3 whether the patient follows treatment and declares seizures.
  • MRD's routine medical letter states that a driver who is seizure-free for six months, compliant with treatment and under regular medical care may hold a Class 5 licence.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Is an EEG required for the NL Neurology Report Form?

Question 4 asks the practitioner to provide an EEG report and results. Question 5 then asks for a description of the nature, cause and circumstances of the seizures, with dates and the effects of other substances, so MRD can see whether seizures were linked to alcohol, drugs or medication changes.

Can a nurse practitioner complete the Neurology Report Form?

Yes. The form is addressed to the driver's physician or nurse practitioner, and the signature block asks for the name and address of the physician or NP, an office telephone number and the date of examination. The driver also signs to authorize release of the information and certify it as correct.

Does the form cover neurological conditions other than epilepsy?

Yes. Question 7 asks about any other current or past neurological disorder, with diagnosis, treatment and relevant dates. Question 8 then asks whether the condition or its treatment affects the patient's ability to operate a motor vehicle, which is the opinion MRD relies on.
