Good to know

  • Release to regular duty without restrictions, or total disability, must be documented on this form or the employer's standard form.
  • A provider who has discussed restrictions by phone with the employer's representative, or completed the employer's own work status form, can skip the restriction portion.
  • Flag medications that limit the employee's ability to work safely, with an explanation, and give the date restrictions end or the recheck date.
  • Attending physicians' reporting duties are set out in Regs. Conn. State Agencies 31-279-9. The form is fillable and dated Rev. 9-26-2011.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How fast must the Connecticut work status form be sent?

The form says to fax a copy to the carrier, TPA, employer or designee within one business day of the visit. A copy is also given to the employee at the visit and another is filed in the medical record.

Can the employer's own work status form be used instead?

Yes, for the restriction portion. A provider who completed the employer's work status form, or discussed restrictions by phone with the employer's representative, need not complete the restrictions here. Release to full duty or total disability must still be documented on this form or the employer's standard form.

How are restrictions recorded on the Connecticut form?

For an 8-hour workday the provider checks 1 to 2, 2 to 4, 4 to 6, 6 to 8 hours or none for standing, walking, sitting, bending, climbing, reaching, twisting, crawling, driving and use of feet and hands, plus a lifting limit and right, left or both for repetitive hand and foot use.
