Hawaii WC-2 Physician's Report (Workers' Comp)

Hawaii attending physicians report on a work injury using the WC-2, covering the injury, diagnosis, treatment and work status for the employer or carrier and the Disability Compensation Division (DCD). Under HRS 386-96 and HAR 12-15-80, the initial report is due within 7 days of the first service, followed by interim and final reports. DCD now takes the WC-2 only through portal access.

Who completes it

The attending physician completes the whole report. The employer reports the injury separately on Form WC-1.

Submitting and payment

How to submit it
Submit it through DCD portal access, which the forms page lists as the only way to file the WC-2; there is no downloadable form. Copies go to the employer or carrier.
Who pays
The DCD forms page sets no fee for the report and does not say who pays for it.

Good to know

  • Older paper versions, such as the 9/05 WC-2 that still circulates on other websites, are no longer accepted.
  • The initial report is due within 7 days of the first service; interim reports and a final report follow as treatment continues.
  • When treatment extends beyond the initial visits, the attending physician also sends the Hawaii Workers' Compensation Treatment Plan to the employer or carrier for approval.
  • DCD lists WC-1, WC-2, WC-21 and WC-36 as the forms that must be submitted through portal access.

How this list is checked

Frequently asked questions

Can I still send a paper WC-2 in Hawaii?

No. The DCD forms page lists WC-2 among the forms that must be submitted via portal access and offers no PDF. Older paper versions found on other websites, such as the 9/05 edition, are no longer accepted, so the report has to go through the portal.

When is the Hawaii WC-2 due?

Under HRS 386-96 and HAR 12-15-80, the initial report is due within 7 days of the first service, with interim and final reports afterward. Copies go to the employer or insurance carrier as well as to the Disability Compensation Division.
