Indiana State Form 2118 Physician's Report (Work Comp)

Indiana's Worker's Compensation Board provides this two-part report for an attending physician or independent medical examiner to document an injured worker's examination. Page 1 records the examination findings and page 2 the permanent partial impairment (PPI) rating. The employer or insurer receives it, and it is filed with the Board when needed to support an agreement to compensation or a PPI dispute.

Who completes it

The attending physician or the independent medical examiner completes both pages. The employer and insurer use it alongside their own filings, such as State Form 1043, Agreement to Compensation.

Submitting and payment

How to submit it
Send the completed report to the employer or insurer, which files it with the Board when an agreement or PPI dispute requires it. The Board does not list it among forms that must go through EDI.
Who pays
The Board's forms page sets no fee for the report and does not say who pays for it.

Good to know

  • Because page 2 is the PPI section, one form carries both the examination findings and the impairment rating once the worker reaches that stage.
  • Related Board forms include State Form 1043, Agreement to Compensation, and State Form 38911, used when temporary total or temporary partial disability benefits end.
  • The Board offers it as a fillable PDF; click the form name or number on the Board's forms page to download it.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can complete Indiana State Form 2118?

Either the attending physician or an independent medical examiner. Page 1 records the examination findings and page 2 the permanent partial impairment rating, so the same form works for a treating report or for an IME report that includes a PPI rating.

Do I file Form 2118 with the Indiana Worker's Compensation Board?

Usually the report goes to the employer or insurer. It is filed with the Board when needed, for example to support an Agreement to Compensation on State Form 1043 or when a permanent partial impairment rating is disputed.
