Montana Medical Status Form (Workers' Comp, 2026)

Montana law (39-71-1036, MCA) requires the treating physician or a designee to complete a Medical Status Form after every office visit with an injured worker. It supports stay-at-work and return-to-work planning by recording current work capacity, modified work abilities, MMI and impairment rating status, and follow-up timing. DLI offers three versions of increasing detail, each meeting the statutory minimum, and the provider completes one per visit.

Who completes it

The treating physician or designee completes the whole form. Treating physicians under 39-71-116, MCA include physicians, chiropractors, physician assistants, dentists, APRNs and physical therapists.

Submitting and payment

How to submit it
The provider's office makes sure the injured worker, the employer and the claims examiner each get a copy after every visit.
Who pays
The form sets no fee and does not say who pays for completing it.

Good to know

  • Version 1 uses frequency boxes from continuous to never, Version 2 hours per day and safe weights, Version 3 free-text restrictions covering activity, frequency, duration, sidedness and force.
  • Choosing modified duty or limited hours triggers Section 3, and a blank activity field means no restriction.
  • Check the box to request the worker's job description; the employer or insurer should then send it so restrictions can be specific.
  • Providers may build the form into their EHR if the form name stays the same and all data fields are included. Effective January 10, 2026.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Is the Montana Medical Status Form available in Spanish?

Yes. DLI publishes Spanish versions of all three forms and the instructions; Version 1 is titled Formulario de Estado Médico. Which version to use is the treating physician's choice, but only one version should be completed after each visit.

Can the Montana Medical Status Form be built into an EHR?

Yes. DLI allows providers to program the form into their electronic medical record as long as the form name stays the same and all data fields are included, so the copies sent to the worker, employer and claims examiner carry the required content.

Can I request an IME or impairment rating on the form?

Yes. Section 4 has checkboxes to request an independent medical evaluation or an impairment rating, next to the MMI fields where you mark whether the worker has reached MMI, and if not, when you expect it.
