Kentucky CMB-MR-2: Continuation of Medical Benefits

Since Kentucky's 2018 change to KRS 342.020, medical benefits for a work injury or occupational disease end after a set period unless continued. To support an employee's application to continue them, the treating physician completes this one-page report stating whether further treatment is reasonably necessary, explaining why, and describing that treatment in general terms. It is filed with Form CMB-APP-1.

Who completes it

The treating physician completes and signs the whole form, adding medical specialty and address. The employee files the CMB-APP-1 application that it supports.

Submitting and payment

How to submit it
Attach it to the employee's Form CMB-APP-1, Application for Continuation of Medical Benefits, filed with the Department of Workers' Claims.
Who pays
The form sets no fee and does not say who pays for the physician's report.

Good to know

  • The key question is a yes or no opinion on whether continued medical treatment is reasonably necessary for the work injury or occupational disease, with an explanation.
  • Only a yes answer needs the second part: a general description of the reasonably necessary treatment related to the injury or disease.
  • One page and not fillable; the printed subtitle reads Medical Report for Continuation of Benefits, Revised August 2018.
  • Companion forms are CMB-APP-1, the application itself, and CMB-ADD-3, the change of address form.
