Alaska Fishermen's Fund Physician's Report 07-6126

The Alaska Commercial Fishermen's Fund, a state program administered by the Division of Workers' Compensation, pays benefits for injured commercial fishermen. Before it approves any benefit payment, 8 AAC 55.020(a)(2) requires this physician's report. It covers exam and treatment dates, hospitalization, treatment provided, diagnosis, whether fishing caused or aggravated the condition, and any pre-existing impairment.

Who completes it

The attending physician completes items 1 to 22 and signs, adding facility name and tax ID. The fisherman files the Fund's own claim form (07-6125) separately.

Submitting and payment

How to submit it
Send the report to the Fishermen's Fund by mail, fax or email using the contact details printed on the form; chart notes can be attached for treatment and diagnosis.
Who pays
The Fund will not approve provider bills until it has received this physician's report.

Good to know

  • The Fund is a state program for commercial fishermen run by the Workers' Compensation Division; it is not workers' compensation insurance as such.
  • Item 13 asks whether the condition was caused or aggravated by commercial fishing, with an explanation; item 14 asks about concurrent or pre-existing injury, disease or impairment.
  • Related forms are the Fishermen's Fund Claim Form (07-6125) and Request for Release of Information (07-6133). The current form is revised 07/2010.
  • The form warns that false information is a crime, and the Fund may deny all benefits if the claimant gave materially false information.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. Frequently asked questions

Why does the Fishermen's Fund need a physician's report?

Alaska regulation 8 AAC 55.020(a)(2) requires the Fund to receive a physician's report of treatment before it approves benefit payments. Until Form 07-6126 arrives, provider bills are not approved, so sending it promptly keeps both the fisherman's benefits and the provider's payment moving.

Is the Fishermen's Fund the same as workers' compensation?

No. The Alaska Commercial Fishermen's Fund is a separate state program for commercial fishermen, administered by the Division of Workers' Compensation. It has its own claim form (07-6125), its own release of information form (07-6133) and this physician's report.

Can chart notes replace the narrative on Form 07-6126?

Partly. For treatment provided (item 11) and diagnosis (item 12) the form says to give details or attach chart notes. The physician still answers the causation question, the pre-existing condition question and hospitalization items, and signs the form.
