NC WIC Program Exchange of Information Form

North Carolina health care providers use this fillable form to share a patient's measurements, lab values and diagnoses with the local WIC program. WIC uses the provider's height or length, weight, hemoglobin and nutrition-related diagnoses when certifying the patient and assessing nutrition risk. It is separate from the NC WIC Medical Documentation form used for special formula.

Who completes it

The patient's health care provider or office staff complete the measurements, lab values and diagnoses and sign the form.

Submitting and payment

How to submit it
Fax the completed form to the patient's local WIC clinic or give it to the family to bring to their WIC appointment. NC DHHS recommends downloading the file to use it as a fillable form.

Good to know

  • The form is posted on the NC DHHS WIC health care provider resources page with the Medical Documentation form and the exempt infant formulas and WIC-eligible nutritionals list.
  • To refer a family not yet enrolled, NC WIC offers an online NC WIC Families Referral Form on the same page and asks providers to encourage patients to apply.
  • Questions about the form go to the local WIC director or the state WIC program, which NC DHHS lists as contacts for health care providers.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What is the NC WIC Exchange of Information form used for?

It lets a health care provider share the measurements, lab values and diagnoses WIC needs, such as height or length, weight and hemoglobin, with the patient's local WIC clinic. That information supports the patient's WIC certification and nutrition assessment. Requests for special formula use the separate NC WIC Medical Documentation form.

How do I refer a new family to North Carolina WIC?

NC DHHS offers an online NC WIC Families Referral Form on its health care provider resources page and asks providers to encourage patients to apply. The Exchange of Information form can then carry the patient's measurements and lab values to the local WIC clinic.
