NC WIC Program Medical Documentation Form

North Carolina WIC requires this documentation before it issues exempt infant formula, WIC-eligible nutritionals or non-contract formula, or modifies WIC foods, for a participant with a qualifying medical condition. The prescriber records the diagnosis or ICD-10 code, the product, amount and duration, and any foods to omit, as federal WIC rules at 7 CFR 246.10(d) require.

Who completes it

A licensed provider with prescriptive authority, such as an MD, DO, NP or PA, completes the whole form and signs it.

Submitting and payment

How to submit it
Fax the form to the patient's local WIC clinic or give it to the family to bring to their WIC appointment.

Good to know

  • Check the requested product against NC WIC's list of exempt infant formulas and WIC-eligible nutritionals, posted on the same NC DHHS health care provider resources page.
  • Measurements and lab values for certification go on the separate fillable WIC Program Exchange of Information form rather than on this one.
  • To connect a family that isn't yet on WIC, NC DHHS provides an online NC WIC Families Referral Form on the same page.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.
