New Hampshire WIC Medical Documentation Form

New Hampshire WIC asks health care providers to document children's measurements and blood work on this one-page form so families can supply the information required for WIC participation. After a parent signs the release, the office records length or height, weight, hemoglobin or hematocrit and lead for up to three children, then sends the form to the local WIC agency.

Who completes it

The parent or guardian signs the authorization at the top, naming each child and date of birth. The provider's office records each child's measurement date and results, the provider's name and clinic, and the staff member completing the form.

Submitting and payment

How to submit it
Email or fax it to the local WIC agency for the family's county; the form lists four agencies covering Carroll and Strafford, Hillsborough and Rockingham, Cheshire and Sullivan, and Belknap, Coos, Grafton and Merrimack counties.

Good to know

  • Measurements can be entered in inches or centimeters and pounds or kilograms, and either a hemoglobin or a hematocrit value can be given.
  • The parent's release lets the provider and WIC staff discuss the request and states that the information will not be shared without consent, except as regulations require, or affect WIC eligibility.
  • WIC serves low-income women, infants and children up to age 5 who have a nutritional risk, and WIC staff complete the nutrition assessment.
  • Requests for standard or special formula use separate New Hampshire WIC forms. The current version of this form is dated 2/2025.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What does the New Hampshire WIC medical documentation form ask for?

For each child, the measurement date, length or height, weight, hemoglobin or hematocrit, and lead level. The provider's name, medical office, contact details and the staff person completing the form go at the bottom. A parent or guardian signs the release at the top first.

Where do I send the New Hampshire WIC medical documentation form?

To the local WIC agency serving the family's county, by email or fax. The form names Goodwin Community Health Center, Community Action Partnership, Southwestern Community Services, and Community Action Program Belknap-Merrimack Counties, each with the counties it covers.

Is this form used to request special formula from New Hampshire WIC?

No. It documents measurements, hemoglobin or hematocrit, and lead. New Hampshire WIC posts separate forms on its health care provider page to request standard formula or special formula and foods, and special or exempt formulas require a documented nutrition-related ICD-10 diagnosis.
