Washington WIC Medical Documentation DOH 961-135

Washington WIC introduced this one-page fillable form on July 14, 2025 for participants who need a premature, hypoallergenic or other WIC formula or nutritional drink, or a change to WIC foods, because of a qualifying condition. The provider records growth data, formula type, amount and form, whether a prescription went to a pharmacy, foods to allow or withhold, a duration of up to six months and the diagnosis.

Who completes it

A health care provider with prescriptive authority (MD, DO, PA, NP or ARNP, or ND) completes sections 1 through 6 and signs. The caregiver signs section 7, a release that lets WIC staff talk with the provider.

Submitting and payment

How to submit it
Fax the form to the participant's WIC clinic or give it to the caregiver to bring to WIC.

Good to know

  • WIC accepts other documentation only if it includes the product name, amount, qualifying diagnosis, validity period, provider contact information, signature and date.
  • Section 2D asks whether the prescription was submitted to a pharmacy, with its name, or whether the formula is not covered by insurance.
  • Diagnoses are listed with WIC risk codes, for example 142 preterm or early term birth, 134 failure to thrive and 353 specific food allergy.
  • The caregiver's release lasts for the current WIC certification and excludes sexually transmitted infections, mental health concerns and substance abuse.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How long is a Washington WIC medical documentation form valid?

For the period the provider specifies, up to a maximum of six months. Section 4 offers 1 to 6 months or an expiration date no more than six months out, and Washington WIC's prescriptions page lists the validity period among the elements every prescription must include.

Can I send Washington WIC a prescription instead of DOH 961-135?

Yes, if it contains all the required elements: the formula or food name, amount, qualifying diagnosis, how long it is valid, the provider's contact information, signature and date. Washington WIC accepts other documentation only when every one of these is present.

Who can sign the Washington WIC medical documentation form?

A health care provider with prescriptive authority. Section 6 lists MD, DO, PA, NP or ARNP, and ND. The caregiver also signs section 7, authorizing WIC staff to discuss the patient's health and nutrition needs with the provider for the length of the current certification.
