Tennessee WIC Therapeutic Formula Request PH-4077

Tennessee WIC uses this one-page request when a participant with a qualifying condition needs a therapeutic formula or WIC-eligible nutritional, along with any changes to supplemental foods. The provider names the formula, the months of issuance and the daily amount, checks the qualifying condition, and can omit foods or request whole or 2% milk with a medical reason. Every request is subject to WIC approval.

Who completes it

A provider (MD, DO, PA-C or NP) completes the required patient, formula and diagnosis sections and signs or stamps the form. If the food section is left blank, the WIC competent professional authority decides the foods.

Submitting and payment

How to submit it
Fax the completed form to the WIC clinic or have the patient return it to their WIC clinic; the fillable version is linked from the Tennessee WIC web page.

Good to know

  • Formula can be issued for up to 6 months, and the documentation must be updated every six months.
  • Write a specific amount: writing in max is not accepted, and children and women cannot exceed 30 ounces a day, the USDA maximum.
  • Colic, constipation, spitting up and gas are not qualifying conditions; accepted examples include FPIES, tube feeding, oral motor feeding issues and low maternal weight gain.
  • An older form, PH-4234 WIC Medical Request for Formula/Foods, still circulates on tn.gov, but the WIC page links PH-4077.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
