Tennessee WIC Referral Form PH-3151

WIC applicants in Tennessee must be found to have a medical or nutritional risk, and the WIC assessment uses current measurements and periodic blood screening. This one-page referral lets a provider send those values with the patient's details: height or length, weight, hematocrit or hemoglobin with their dates, the estimated delivery date for a pregnant patient, primary language, and any nutrition or health concerns.

Who completes it

The health care provider's office completes the patient and provider sections, which the form marks to be completed by the health care provider, and the physician or a designee signs.

Submitting and payment

How to submit it
Fax or email the completed referral to the patient's local health department or WIC office; office contacts are listed on the Tennessee WIC web page.

Good to know

  • Record a date beside each height or length, weight and hematocrit or hemoglobin value; the form asks for a date with every measurement.
  • The signature line reads physician's (or designee) signature, so the referral doesn't have to be signed by the physician personally.
  • Formula requests need a different form, the Request for WIC Therapeutic Products and Supplemental Foods, PH-4077.
  • Tennessee WIC posts its Nutrition-Related Qualifying Conditions document (2019) next to this referral in its section for health care providers.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
