New Mexico WIC Medical Referral Form

This one-page form lets a New Mexico clinician pass a patient's height, weight, hemoglobin or hematocrit and nutrition concerns to WIC, so the program can document the medical or nutritional risk needed for enrollment. It has separate blocks for pregnant women, breastfeeding and postpartum women, infants and children under 24 months, and children 2 to 5. Sending it helps WIC avoid repeating bloodwork and measurements.

Who completes it

The health care provider completes the patient information and the block that matches the patient, checks any conditions, and signs or stamps it (MD, DO, PA or CNP) with an NPI.

Submitting and payment

How to submit it
Return the completed form to the closest WIC clinic; the form says fax is acceptable, and families can also bring it to their WIC appointment.

Good to know

  • Measurements must be recent: no older than 60 days for women and children 2 to 5, and within 30 days for the weight and length of infants and children under 24 months.
  • Pregnant women need a hemoglobin or hematocrit from the current pregnancy, and breastfeeding or postpartum women need one taken in the postpartum period.
  • The referral is not mandatory, and the form asks you to refer the patient even if no condition box is checked; WIC staff may call for more detail.
  • Formula or food requests use a separate form, the New Mexico WIC Medical Request for Formula and/or Food.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Is the New Mexico WIC Medical Referral Form required?

No. New Mexico WIC's health care provider page says the referral is not mandatory, but it helps WIC document the risk factor needed for enrollment, avoid repeating bloodwork and measurements, and give better counseling and referrals. The form asks providers to refer patients even when none of the condition boxes apply.

How recent do measurements need to be on the NM WIC referral?

Height and weight for women and for children 2 to 5 must be no older than 60 days. For infants and children under 24 months, weight and length must be within 30 days and hemoglobin or hematocrit within 60 days. Pregnant women need a value from the current pregnancy.

Who signs the New Mexico WIC Medical Referral Form?

A health care provider signs or stamps it and marks MD, DO, PA or CNP, then adds the date, printed name, NPI, phone and fax. The provider block is labeled required, as is the patient information block at the top of the form.
