New Mexico WIC Medical Request for Formula and Food

New Mexico WIC needs this request on file before a family receives any medically requested formula or food, including exempt or medical formulas and changes that add or remove regular WIC foods. The back of the form lists every exempt or medical formula NM WIC provides. Some products are bought at WIC-approved stores, while others are ordered from a third-party vendor and delivered to the WIC clinic.

Who completes it

A provider legally authorized to prescribe in New Mexico completes it: an MD, DO, nurse practitioner, certified nurse-midwife, physician assistant supervised by a physician, or registered dietitian.

Submitting and payment

How to submit it
The completed request must reach the WIC clinic before the family receives the formula or food; families can bring it, or the office can fax it to the local clinic. New Mexico WIC lists the form on its health care providers page.

Good to know

  • If the form isn't available, NM WIC may accept a prescription showing the date, patient name and birth date, qualifying diagnosis, product, and provider signature and contact information.
  • Daily amount and length of use are helpful but not required on a prescription; the maximum length allowed is 6 months.
  • While a vendor-ordered formula is on its way, the provider may need to help the family with a temporary arrangement such as samples or an alternate product.
  • Providers can ask NM WIC to consider covering a formula or food that is not currently on its list.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Can a prescription replace the New Mexico WIC Medical Request form?

Yes. NM WIC may accept a prescription that includes the prescription date, the participant's name and date of birth, the qualifying medical condition, the formula or nutritional supplement, and the provider's signature and contact information. The amount per day and length of time are helpful but not required.

How long can a New Mexico WIC formula request last?

Up to 6 months. NM WIC lists the length of time required among the prescription details and sets a maximum of 6 months, so a patient who needs a medical formula for longer will need a new request from the provider.

Can a registered dietitian sign a New Mexico WIC formula request?

Yes. New Mexico WIC accepts prescriptions from providers legally authorized to prescribe in the state, and its list includes registered dietitians alongside MDs, DOs, nurse practitioners, certified nurse-midwives and physician assistants supervised by a physician.
