Nebraska WIC Provider Authorization: Children & Women

Nebraska WIC needs this authorization before it issues a specialty formula, whole milk after age 2 or 2% milk at age 1 to a child, or a specialized formula or whole milk to a pregnant, breastfeeding or postpartum woman. The provider records the diagnosis and growth data, the formula and amount, foods to withhold and the months requested. WIC cannot issue anything until every applicable section is complete.

Who completes it

A licensed provider (MD, DO, PA or NP) completes sections A through G, including the required diagnosis and clinical data, and signs or stamps section G. The WIC clinic fills in its contact block at the top and the approval line at the bottom.

Submitting and payment

How to submit it
Fax the form to the patient's local WIC clinic named at the top, or give it to the family to bring to their WIC appointment.

Good to know

  • If the child receives Medicaid, the form asks you to send the formula prescription to the managed care organization; Nebraska infants and children on Medicaid get specialty formula from Medicaid.
  • Whole or 2% milk and pureed food substitutions are only available to patients who also receive a specialty formula and have a medical need; personal preference does not qualify.
  • Leaving section F blank gets six months of issuance, and leaving section D blank means every WIC food is issued.
  • Page 2 lists example qualifying diagnoses with ICD-10 codes, such as celiac disease K90.0 and hyperemesis gravidarum O21, and excludes picky eating, poor appetite and nonspecific intolerance.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Does a Nebraska child need a Provider Authorization Form for contract formula?

Yes, for children ages 1 to 5. Nebraska WIC's provider page says a PAF is needed for any contract or specialty formula on the Formulary for a child, and also for whole milk over age 2 or 2% milk at age 1. For infants, contract formulas need no PAF.

How much formula does Nebraska WIC provide for a child or woman?

Section C notes that WIC provides approximately 29 ounces per day. Check maximum allowable or enter a specific number of ounces per day, and add any special instructions. If the child receives Medicaid, the form asks you to send the prescription to the Medicaid managed care organization instead.

Who can sign the Nebraska WIC Provider Authorization Form?

Section G takes the signature or stamp of a health care provider with the credentials MD, DO, PA or NP, along with the date, printed name, phone and fax. WIC staff then record the participant's family ID, who approved the request and the approval date.
