Nebraska School Meal Medical Statement (NDE)

Nebraska schools use this Department of Education statement when a student's meal accommodation cannot be met within the federal school meal pattern requirements. The licensed professional describes the physical or mental impairment that restricts the diet, lists foods to omit and substitute, and notes any texture or thickness changes. The family returns it to the school, which logs the date received and any follow-up.

Who completes it

A state-licensed health care professional completes and signs the medical section; the form names physicians (MD or DO), physician assistants, APRN nurse practitioners, chiropractors and registered dietitians. The parent or guardian fills in the child, contact and school details, and the school completes its internal-use box.

Submitting and payment

How to submit it
Give the signed statement to the parent or guardian, who returns it to the school; the school's internal section records where it goes, the date received and any follow-up.

Good to know

  • Requests that can be met within the meal pattern go on the separate Request for Meal Accommodation, which a parent or guardian may complete without a medical signature.
  • A parent or guardian may sign this form only for a milk accommodation at lunch; schools serving only lunch can use the separate Medical Statement for Alternative Milk at Lunch.
  • The current template is an editable Word (.docx) file dated 7.2026, with a Spanish version posted next to the English one.
  • The form applies to schools serving breakfast, lunch or snack and cites USDA memo SP 59-2016 on accommodations that fall outside the meal pattern.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign the Nebraska school meal Medical Statement?

The form must be completed by a State Licensed Health Care Professional. It names physicians (MD or DO), physician assistants, APRN nurse practitioners, chiropractors and registered dietitians. The one exception is a milk accommodation at lunch only, where the child's parent or guardian may sign instead.

When does a Nebraska school need a Medical Statement instead of a parent request?

Only when the accommodation cannot be made within the federal meal pattern requirements for school meals. Changes that still meet the meal pattern go on the Request for Meal Accommodation, which a parent or guardian may complete without a clinician's signature.

Is the Nebraska Medical Statement available in Spanish?

Yes. The Nebraska Department of Education posts a Spanish Word version of the Medical Statement next to the English one on its special diets page. Both are editable .docx templates, so the clinician can type the impairment, foods to omit and substitute, and texture or thickness changes before signing.
