South Carolina WIC Medical Referral Form D-4113

South Carolina WIC uses this referral to collect accurate, current measurements and medical history from a patient's primary care physician or pediatrician. Patients often bring it to be filled in before their next WIC appointment, and offices also use it to refer a new client. It covers women, infants and children under 24 months, and children 2 to 5, with a checklist of diagnoses and concerns.

Who completes it

A health care provider completes and signs it, using a separate form for each patient. Only the patient's name and date of birth are required in section A; the provider fills the matching block in section B, checks concerns in section C and signs section D.

Submitting and payment

How to submit it
Give the completed form to the patient or parent to bring to the WIC appointment, or fax it to the local WIC agency, which scans it into the SCWIC system.

Good to know

  • Children 2 to 5 need hemoglobin or hematocrit yearly if normal and every 6 months if abnormal; include a venous lead value and immunization status when available.
  • Check Specific Diet Prescription if you want a WIC nutritionist to counsel the patient on a particular diet; WIC staff may call for more detail.
  • Special formula requests go on the separate SC WIC Medical Documentation Form for Formula, D-2074.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
