South Carolina WIC Form D-2074: Special Formula

South Carolina WIC requires this documentation for infants and other participants who need a special medical formula rather than the Similac contract formulas. The prescriber checks ICD-coded diagnoses, adds growth data, names the formula, amount, length of use and physical form, and chooses how WIC supplemental foods are handled. Health departments order approved special formulas, and delivery can take seven or more days.

Who completes it

A health care professional licensed to write prescriptions under South Carolina law completes and signs all five sections, filling in every provider box. WIC staff record the participant ID.

Submitting and payment

How to submit it
Fax it to the local WIC agency or give it to the family to bring to WIC; the clinic scans it into the SCWIC system under the participant's record.

Good to know

  • Prescriptions cannot exceed 6 months or run past the infant's first birthday; metabolic formula prescriptions are the exception and may run up to one year.
  • Enfamil AR needs two documented diagnoses supported by growth data: GERD plus a related condition such as failure to thrive, frequent pneumonia or prior GERD surgery.
  • Spitting up, formula intolerance, picky eating, constipation, fussiness and gas are not accepted, and WIC won't supply formula just to boost intake or manage weight.
  • Anthropometric data in section 2 is required whenever the diagnosis is weight-related, such as failure to thrive or prematurity.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How long can a South Carolina WIC special formula prescription last?

Up to 6 months. Section 3 offers 1 to 6 months, or up to the infant's first birthday without exceeding 6 months. The instructions make one exception: a prescription for metabolic formula may run up to one year.

What does South Carolina WIC need to approve Enfamil AR?

Two medical diagnoses, supported by anthropometric data. One must be GERD, and the other must be a related condition such as a history of GERD surgery like fundoplication, failure to thrive, weight loss or inadequate weight gain, or frequent pneumonia.

Why can a South Carolina WIC special formula take time to arrive?

Approved special formulas are not contract formulas, so the health department orders them, and the form warns that delivery can take seven or more days. The prescription also remains subject to WIC approval under program policy, so check the SC WIC approved formula list before writing it.
