Alabama WIC 114 Hospital Special Formula Notification

When a WIC infant or child leaves the hospital on a special medical formula, this notification lets Alabama WIC order the product around discharge instead of waiting for a clinic visit. It records the diagnosis or ICD-10 code, formula, mixing instructions, 24-hour volume, discharge date and allowed foods, and then serves as the participant's prescription.

Who completes it

Hospital staff complete it: the hospital dietitian signs, and the form also names the health care provider, the hospital or clinic and the parent or guardian. A State WIC staff section records when the form was received, when formula was ordered and when it went to the clinic.

Submitting and payment

How to submit it
The hospital dietitian faxes the completed form to the State WIC Office, which places the order for the exempt infant formula or WIC-eligible nutritional and then faxes the form on to the participant's WIC clinic.

Good to know

  • Height and weight, if provided, must have been measured within the last 60 days. Hemoglobin or hematocrit and the date it was done can also be entered.
  • USDA allows supplemental foods for infants and children on special formula only if prescribed. WIC cannot provide a food unless it is checked, so mark 'formula only' or each allowed food.
  • If no food is prescribed, infants 6 to 12 months on a non-contract formula may receive 29 fluid ounces of formula a day.
  • The 114 covers the discharge only. A regular ADPH-WIC-111a (infant) or 111b (child) prescription is needed at the 6-month and 12-month evaluations.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Does the WIC 114 replace a formula prescription?

For the discharge period, yes. ADPH states the form serves as the participant's prescription and is scanned into the participant's WIC record. Intended length of use can be 1 to 6 months, but the 6-month and 12-month evaluations need a standard 111a or 111b prescription from the community provider.

What if the patient is not yet enrolled in WIC?

Item 5 asks whether the participant is currently on WIC, with options for yes and where, do not know, or no and the clinic the family was referred to. Item 6 asks whether the mother was on WIC during pregnancy. The State WIC Office then forwards the form to the appropriate clinic.
