Alabama WIC 111b Child/Woman Formula Prescription

Children 12 months and older and women in Alabama WIC need this prescription to receive any formula or WIC-eligible nutritional product. The prescriber gives a nutrition-related diagnosis, the product, daily amount and duration, and marks any WIC foods to remove. A WIC dietitian or nutritionist then approves issuance under program policy.

Who completes it

A physician, or a PA or nurse practitioner working under a physician's orders, completes and signs the form. WIC clinic staff record the participant ID, date received and approver.

Submitting and payment

How to submit it
Fax the prescription to the participant's local WIC clinic or hand it to the patient to take to WIC. Only this form is accepted for special formula requests, and versions from before April 2026 should be discarded.

Good to know

  • Daily amount choices are 8, 16 or 24 ounces, or a written amount. USDA caps issuance at 30 ounces a day, and prescribing that much requires monthly clinic visits.
  • Intended length of use runs from 1 to 6 months. After 6 months a new prescription is required, and if it isn't renewed no formula can be issued.
  • Weight, height, BMI and the measurement date are required when an increased-calorie supplement is prescribed. Federal rules bar formula issued only to enhance nutrient intake or manage weight.
  • If no foods are checked for removal, the WIC dietitian builds the food package. Milk, cheese and yogurt are not issued alongside elemental or amino acid formulas.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Do children over 12 months need a prescription for standard formula in Alabama WIC?

Yes. ADPH states that children 12 months and older and women need a prescription form to receive any formula or WIC-eligible nutritional product. The 111b is that form, and the patient's need must be re-evaluated by the provider every six months, when a new prescription is written.

Which foods can be removed on ADPH-WIC-111b?

The form lists milk or soy milk, cheese, yogurt, eggs, juice, fruit and vegetables, cereal, peanut, nut or seed butter, canned fish, beans or peas, and whole grains. Check every food the patient may not eat while on the special formula; if nothing is checked, the WIC dietitian or nutritionist decides.

Who can sign Alabama's 111b formula prescription?

The instructions define a health care provider as a physician or someone working under a physician's orders, such as a physician assistant or nurse practitioner. The signature, printed name, practice or clinic name and date are all required, and WIC staff complete the approval block at the bottom.
