Washington F252-114-000: Wound Vac (NPWT) Request

Washington L&I uses this two-page form for preauthorization and each 30-day continued authorization of negative pressure wound therapy on a workers' compensation claim. It documents a complete wound therapy program tried and failed, wound location, size, depth, tunneling and drainage, wound type, FDA safety contraindications and recent labs. Current dated wound photos and the physician's prescription must accompany it.

Who completes it

The nursing staff caring for the worker, at a nursing facility, nursing service or home health agency, complete the entire form for preauthorization and sections I, III and V for each 30-day review. The prescribing provider is identified in section I.

Submitting and payment

How to submit it
Fax it to L&I's Occupational Nurse Consultants at the number on the form, with the photos, the prescription, notes from the last two visits and two wound care notes.
Who pays
The form lists no billing code; it secures authorization for the therapy, which is reviewed again at the first, second and third 30-day reviews.

The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
