Washington F245-340-000: L&I Prosthetic Device Request

Washington L&I uses this request to decide whether to authorize a new, repaired or replacement prosthesis for an injured worker, one device per request. The clinician documents amputation level and residual limb condition, upper or lower extremity function, K-level and strength for leg prostheses, work and mobility demands, prior devices, and the HCPCS components requested with cost and medical necessity.

Who completes it

The prosthetic clinician completes all sections, names the attending provider, signs, and attaches residual limb measurements and warranty information. L&I's Occupational Nurse Consultant completes the authorization block.

Submitting and payment

How to submit it
Send it to L&I by fax or mail as printed on the form, with the client's name and claim number on every attached page.
Who pays
L&I reimburses authorized prosthetic devices according to its fee schedule, and the form lists the HCPCS codes, quantities and cost of each requested component.

Good to know

  • Microprocessor knees and myoelectric upper limbs need the matching addendum; the knee addendum asks about K3 or K4 level, a successful trial, skin integrity and wearing tolerance of at least 6 hours a day.
  • K-levels follow the Medicare classification, from K0 (no ability to ambulate or transfer safely) to K4 (high impact, stress or energy levels typical of an active adult).
  • Answer every question in sections 1, 4 and 5, entering NA where one does not apply, and note bilateral amputations under Accepted Diagnosis.
  • A Spanish version of the request (F245-340-999) is also available from L&I.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Can I request more than one prosthesis on L&I's form?

No. The form header states one prosthetic device per request. A secondary prosthesis or a replacement needs its own request, and section 5 asks for the necessity and purpose of a secondary device, prior device history and the reason for any change from an exact replacement.

What extra documentation does a microprocessor knee request need?

Complete the Microprocessor Knee Request Addendum. It asks whether a physician specializing in amputee rehabilitation made the recommendation, how the K-level was derived, prosthetist access, trial results with a gait description, hip flexion contracture under 20 degrees and agreement to the manufacturer's weight and environment limits.
