Washington F252-095-000: L&I Opioid Treatment Agreement

Washington prescribers and injured workers sign this two-page agreement when chronic opioid therapy is part of treatment on a workers' compensation claim. The worker agrees to take medications as prescribed, use one prescriber and pharmacy where possible, take part in return-to-work efforts, allow random drug tests and pill counts, and keep appointments. It also lists when the provider will stop or change opioid treatment.

Who completes it

The worker reads and signs it, naming the prescriber and pharmacy, and the prescribing provider signs as well. Both acknowledge both sides of the agreement.

Submitting and payment

How to submit it
Keep a signed copy on file, give a copy to the worker and send a copy to L&I. Renew the agreement yearly.
Who pays
L&I's opioid forms payment policy lists billing codes for the chronic (1078M) and subacute (1076M, 1077M) request forms but not for the agreement itself.

Good to know

  • Only one lost prescription or medication will be replaced in a calendar year under the agreement's terms.
  • The provider checks the state prescription monitoring program and may stop or change opioids for lack of functional improvement, misuse, opioids from another provider or refusing a drug test.
  • Page two covers workplace safety risks, side effects, dependence, tolerance, addiction and safe disposal options, so review it with the worker before signing.
  • A Spanish version of the agreement (F252-095-999) is also available from L&I.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

How often should the L&I opioid agreement be renewed?

Yearly. The form tells providers to renew it every year, and the Chronic Opioid Request Form asks whether a treatment agreement was signed and notes it should be renewed yearly. A new agreement is also expected when a new prescriber takes over the worker's chronic opioid therapy.

Is the L&I opioid treatment agreement available in Spanish?

Yes. L&I publishes a Spanish version as F252-095-999. Whichever language is used, the worker signs after reading both sides, the provider signs, a copy goes to the worker and another to L&I, and the signed original stays in the chart.
