VA Stomach and Duodenal Conditions DBQ (Ulcer)

Peptic ulcer disease, chronic and drug-induced gastritis, H. pylori infection, stomach stenosis or injury, Zollinger-Ellison syndrome and the effects of stomach surgery are documented for VA disability claims on this 8-page questionnaire. The clinician records how often multi-day episodes of pain, nausea or vomiting occur on daily medication, bleeding and anemia, post-gastrectomy and vagotomy residuals, and endoscopy, imaging, CBC and H. pylori results.

Who completes it

The Veteran's treating or examining clinician completes the form and signs the Section XII examiner's certification. The Veteran completes no part of the form.

Submitting and payment

How to submit it
The clinician gives the signed DBQ to the Veteran, who uploads it with the claim on VA.gov or mails it to the VA Evidence Intake Center; a representative may submit it on their behalf.
Who pays
VA will not pay or reimburse the cost of completing the stomach and duodenum DBQ; any fee is between the Veteran and the provider.

Good to know

  • GERD and esophageal disorders are excluded by the form's title and go on the Esophageal Conditions DBQ instead.
  • Episodes count when abdominal pain, nausea or vomiting last at least three consecutive days and are managed by daily medication: three or fewer, or four or more in 12 months.
  • Peptic ulcer disease can be diagnosed by upper GI imaging or endoscopy; testing of record that matches the current condition need not be repeated.
  • Functional digestive and GI dysmotility disorders are routed to the Intestinal Conditions questionnaire, and stomach stenosis uses both the ulcer and upper GI surgery sections.
