VA Spina Bifida DBQ (Child of Herbicide-Exposed Vet)

Unlike most DBQs, this 5-page questionnaire documents the claimant child rather than the Veteran: it supports a spina bifida claim for the natural child of a Veteran exposed to herbicides. The examiner records the spina bifida type and surgeries, then picks the level that fits for locomotion, upper extremity function, IQ, urinary continence and bowel continence. VA uses those levels to set the child's monthly allowance.

Who completes it

The claimant's health care provider completes all sections and signs the Section XII examiner's certification with NPI, license and specialty. The claimant and the Veteran parent complete no part of the form.

Submitting and payment

How to submit it
The DBQ is submitted with the child's claim on VA Form 21-0304, uploaded or mailed to the VA Evidence Intake Center.
Who pays
VA does not pay or reimburse any cost of completing the spina bifida DBQ; any fee is between the claimant and the provider.

Good to know

  • If spina bifida occulta is the only type, the rest of the form does not need to be completed; meningocele and myelomeningocele continue through all sections.
  • For each level the examiner states whether an unrelated condition contributes and, if so, whether its effect can be separated from the spina bifida.
  • IQ is reported from records reviewed in bands (90 or higher, 70 to 89, 69 or less, or unknown), naming the document and date it came from.
  • Additional questionnaires and additional diagnostic testing are not required; related disabilities such as blindness, uncontrolled seizures or renal failure are described on this form.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who is the claimant on the VA spina bifida DBQ?

The natural child of a Veteran exposed to herbicides. The form identifies both the Veteran and the claimant with their Social Security numbers, and every question describes the claimant, including whether the claimant can manage benefit payments, with an option for a child under 18.

How are bladder and bowel continence graded on this DBQ?

Urinary continence ranges from continent without medication to unable to stay dry for three hours during waking hours at least three times a week despite treatment. Bowel continence ranges from continent to needing manual evacuation or digital stimulation, absorbent materials at least four days a week, or a colostomy bag.

What locomotion levels does the form use?

Item 3A asks for the claimant's primary means of mobility in the community: walking without braces or other external support, walking with braces or external support, or using a wheelchair. Items 4A to 4C ask whether upper extremity impairment prevents grasping a pen, self-feeding or self-care.
